Location is in Central Florida in Lake Wales, off of State Route 60 at 421 Linden Lane. 

Directions:
Turn off of State Route 60 at the light on 11th Street.
(Lake Wales Hospital is located North on 11th Street)

Turn right on the first street, Carlton Ave., before the Apothecary Shop.

Turn left on the first street, Linden Lane, and drive to the end of the street.

Office is on the right in Dr. Caroline Honculada's building at 421 Linden Lane. Enter the waiting room by the steps or ramp off the parking lot.

Gift Certificates are available. Gift Certificates may be purchased in exact amounts per service or starting at $20 toward a service. Out of area clients may purchase gift certificates by phone to give to friends and family member in the Lake Wales area.

PLEASE, call 24 hours in advance if you need to reschedule your appointment. In consideration for other clients, if you arrive late, the time you are late will be deducted from your appointment.

CHECKS, CASH, VISA and MASTERCARD accepted. $25 will be charged for returned Checks.
